I hereby certify that the statements contained in the application are true, accurate and complete and that I presently meet all eligibility requirements set forth in this application. (NOTE: any false statement in this application shall constitute grounds for revocation or withdrawal of the volunteer program.)
By submitting this Release and Waiver of Liability on behalf of myself or my dependents, I hereby release and hold harmless the Lodi Grape Festival & National Wine Show Association, Inc., the State of California, the County of San Joaquin, their agents, directors, officers, servants and employees from all liability resulting from any injury sustained in connection with and arising out of such hazards, or in connection with my participation in the Lodi Grape Festival Volunteer Program, or my presence on the fairgrounds.
I grant permission to be photographed in connection with the Lodi Grape Festival Volunteer Program. I understand that any photography may be used by the Lodi Grape Festival or authorized representative in television, visual, graphic, electronic, printed or other media. I agree to release, indemnify and hold harmless the Lodi Grape Festival & National Wine Show Association, Inc., the State of California, the County of San Joaquin, their agents, directors, officers, servants and employees with respect to any and all claims related to the usage of such photographs by the Lodi Grape Festival or any other media.
